End-of-Life Guidance
Steady, personalized support through one of life’s most profound transitions.
This service offers compassionate, individualized guidance for people with a terminal diagnosis, families navigating end-of-life care, or anyone seeking grounded support in life’s final chapter. Whether you’re planning ahead or already in the midst of this journey, we walk alongside you with presence, clarity, and care.
Each person’s path is unique. We tailor our support to reflect your needs, energy, and deeply held values. We begin by listening—and shape our work from there. This comprehensive package draws from all our core offerings: Advance Care Planning, Vigil Planning, and Caregiver Respite.
What’s Included
We provide non-medical support tailored to your needs, working in collaboration with your care team and loved ones. This service draws from all our core offerings—Advance Care Planning, Vigil Planning, and Caregiver Respite—and reflects your values, preferences, and priorities.
Support may include:
Emotional and practical support for you and your loved ones
Vigil and after-death planning
Advance Care Planning, including key documents and conversations
Life review and legacy projects (letters, recordings, keepsakes)
Respite for caregivers
Coordination with hospice, palliative care, or other providers
Flexible in-person and virtual visits (in-person in NYC; virtual care nationwide)
Please note - we do not provide medical treatment, medication administration, or personal care. Our role is to enhance—not replace—the care already in place.
Our visits are shaped by your goals, energy, and what’s happening day to day. There’s no script—just steady presence, clear communication, and responsive care. Most clients begin with 1–2 visits per week, each lasting 2–3 hours. As needs shift, visits may increase to 4–5 times per week. We’ll work together to find a rhythm that feels supportive and sustainable.
Depending on your needs, support might include:
Exploring what a peaceful death looks like to you
Facilitating honest conversations with family or chosen loved ones
Creating legacy materials that reflect your life and values
Discussing vigil preferences—music, environment, comfort, rituals
Coordinating logistics and visitor schedules
Offering calm, grounded presence during difficult moments
Acting as a bridge between you and your care team
Giving caregivers space to rest or take a break
We’re here to reduce stress, ease uncertainty, and help everyone feel a little more prepared.
